Tennis Elbow Isn’t Just an Elbow Problem

Tennis elbow, or lateral epicondylitis, is often misunderstood as a simple overuse injury of the forearm. While pain shows up near the elbow, research makes it clear that the issue is far more complex. Studies show that only about 48% of patients improve long-term with rest alone, while nearly 78% experience lasting success with progressive loading-based physical therapy (Vicenzino et al., British Journal of Sports Medicine).


Tennis places high rotational, gripping, and deceleration demands on the body. When strength or control is lacking at the shoulder, trunk, or grip, force is redirected to the elbow. Over time, this leads to tendon overload and persistent pain (Coombes et al., BJSM).

Traditional approaches like rest, braces, or injections may reduce symptoms temporarily, but they often fail to address why the tendon became irritated in the first place. Tendons respond best to gradual, progressive loading, which improves tissue capacity and resilience rather than avoiding stress altogether.
