Is Your Injury Holding You Back From Your Active Lifestyle?
You're not looking for quick fixes—you need solutions that get you back to what you love.
Recurring Injuries Keep Sidelining Your Performance
Cookie-Cutter PT That Doesn't Understand Athletes
You're Wasting Time Without Real Progress
You've tried resting and generic exercises, but the pain keeps coming back every time you push yourself. Without addressing the root cause, you're stuck in a frustrating cycle.
You need more than basic stretches and ice. Generic physical therapy clinics hand you off to assistants who don't get your goals or the demands of your sport.
Rushed 15-minute appointments with multiple patients at once leave you confused about your treatment plan and wondering if you'll ever get back to 100%.

Common Questions
Get answers about our sports physical therapy services
1. What makes your sports physical therapy different from traditional PT?
Every session at Streamline Performance PT is 1-on-1 with a Doctor of Physical Therapy for the full hour—no assistants, no techs, no distractions. We blend expert physical therapy with performance training and use advanced techniques like dry needling, shockwave therapy, and VALD force plate testing. Our physical therapists and strength coaches coordinate your care to ensure seamless recovery and return to sport.
2. How long are physical therapy sessions?
Each physical therapy session is a full 60 minutes, one-on-one with your Doctor of Physical Therapy. This allows time for hands-on manual therapy, therapeutic exercises, and education—all tailored to your specific injury and goals. Unlike clinics that see multiple patients at once, your therapist is focused entirely on you.
3. Do I need a referral to start physical therapy?
Arizona is a direct access state, which means you can see a physical therapist without a physician referral. You can schedule your initial evaluation directly with us. If you're using insurance, we recommend checking with your provider about any referral requirements for coverage.
4. What should I expect during my first visit?
Your first visit includes a comprehensive evaluation with your Doctor of Physical Therapy. We'll discuss your injury history, goals, and current symptoms. You'll undergo movement screens, functional testing, and—when appropriate—force plate analysis to identify the root cause of your condition. We'll create a personalized treatment plan and often begin hands-on treatment during the first session.
5. Can you help with running injuries and return-to-running programs?
Absolutely. We specialize in running-related injuries including IT band syndrome, plantar fasciitis, shin splints, Achilles tendinopathy, and runner's knee. We offer running gait analysis, running form assessment, and progressive return-to-running protocols. Our goal is to get you back to running pain-free and stronger than before.
6. Do you offer pelvic floor physical therapy for athletes?
Yes. Dr. Krista Zelizney and Dr. Jake Lape specialize in pelvic floor therapy for both women and men. This includes treatment for pelvic pain, incontinence, postpartum recovery, diastasis recti, and core dysfunction. Pelvic floor therapy is especially important for runners, CrossFit athletes, and postpartum athletes returning to high-impact activities.
